Run clang-format over the Mosh source tree. This is a large change and
has been factored into its own commit for auditability. Reproduce it
with

Removed shared_ptr shim

Since C++17 is now the default mosh version, remove the shared_ptr
shim in favor of std::shared_ptr.

Switch to fully-qualified #include
...
Previously, mosh used extensive -I flags and all of the mosh-local
makes it really hard to tell what the proper dependency graph is, so
instead remove the -I arguments in favvor of $(top_srcdir) and qualify
the paths wherever they are used.

Allow non-inserting prediction.
...
This may be useful for users who find prediction's activity right of
the cursor distracting.
Prediction underscoring is still a little weird sometimes, it replays
a history of known/unknown changes as acks come in from the server.

mosh-client: Make terminal escape character configurable

Uses environment variable MOSH_ESCAPE_KEY. Defaults to current Ctrl-^
which is somewhat problematic for many non-US keyboards.
